Non-microbial etiology: foreign body reaction maintaining post-treatment apical periodontitis

چکیده
The polymerase chain reaction (PCR) is an elegant technology for faithfully replicating and amplifying the master molecule of life, but a valid scientific procedure remains essential without which the resulting data have only very limited value. The presence of microbial infection in the complex apical root canal system is the major cause of posttreatment apical periodontitis in well-treated teeth. However, in rare cases, non-microbial etiological factors, located beyond the root canal system (within the inflamed periapical tissues), can maintain the disease in root-filled teeth. These factors include foreign body reaction to exogenous materials or endogenous cholesterol crystals, a cystic condition of the lesion and extraradicular actinomycotic infections. This article addresses foreign body reaction at the periapex, as a pathobiological factor that maintains post-treatment apical periodontitis.

On the causes of persistent apical periodontitis: a review.
Apical periodontitis is a chronic inflammatory disorder of periradicular tissues caused by aetiological agents of endodontic origin. Persistent apical periodontitis occurs when root canal treatment of apical periodontitis has not adequately eliminated intraradicular infection. Problems that lead to persistent apical periodontitis include: inadequate aseptic control, poor access cavity design, m...
متن کاملSurvival against the odds: microbiology of root canals associated with posttreatment disease
Most periapical lesions resolve after root canal treatment of teeth with primary infections. Over the last decade there has been a renewed focus on post-treatment apical periodontitis and its etiology. This review describes the microbiota associated with persistent post-treatment infection, including microbial identification, ecology, and environmental selection. Compared with untreated teeth, ...
متن کاملApical periodontitis
Apical periodontitis is an inflammatory lesion in the periodontal tissues that is caused mostly by bacterial elements derived from the infected root canal system of teeth (Core concept 7.1). In non-treated teeth apical periodontitis represents a defensive response to a primary infection in a necrotic pulp. Apical periodontitis may also develop due to a secondary infection subsequent to endodont...

Bacterial sampling of prepared root canals is used to determine the presence and character of the remaining microbiota. However, it is likely that current sampling techniques only identify organisms in the main branches of the root canal system whereas it is unlikely that they can sample areas beyond the apical end-point of preparation and filling, or in lateral canals, canal extensions, apical...

INMICROSCOPY, AS INNATURE,ONERECOGNIZESONLYWHATONEALREADYKNOWS. Routine histopathological diagnostic reports and publications based on retrospective reviewing of such, perpetuate the notion that nearly half of all apical periodontitis lesions are cysts. Studies based onmeticulous serial sectioning of apical lesions retrieved in toto show that the actual prevalence of cysts is only about 15% of ...
